Camp Process

Steps to do

1. Athlete must fill out the registration form as well as the medical form
2. Athlete must read and sign the liability waiver form
3. Athlete must read the camp rules
4. Athlete can print out the form, fill out and email all the forms using PDF   to filetsports@gmail for approval
5. Athlete must wait for email approval before making payment
6. Athlete must pay before soccer camp in June 26^th, 2015.
7. Athlete must print his ticket once payment is made and bring it during camp

**Entry is limited and based on a first come first served basis

** Must be a male between the age of 15 and 23.

Admission: $60 USD

* Campers will receive: Lunch, professional formation, and information
